As a Bluecrew worker, I understand that I must comply with the Employbridge LLC and Bluecrew Inc. (hereafter referred to as "the Bluecrew") Drug and Alcohol Policy.
If a negative drug test is a requirement of placement in the position for which I am being considered, I understand that I may refuse to take the test if I wish.
I understand that if I consent to testing:
I will have to supply a specimen at an authorized collection site I choose on Bluecrew App and cooperate in the normal collection procedures.
My specimen will be tested by a designated testing agent chosen by the authorized testing location for alcohol and/or illegal drugs (as defined by the Bluecrew’s Policy) and any other controlled substances as dictated by the circumstances and in accordance with the requirements of applicable law.
If the designated testing agent finds evidence of drug use in my specimen and I do not agree with the positive test results, I will be offered the opportunity to re-test at an approved testing facility. If the test result at the testing facility is positive, a Medical Review Officer (“MRO”) will make reasonable efforts to contact me so I can attempt to explain or rebut my test results. If I explain or rebut the results to the satisfaction of the MRO, I will be treated as if I passed the test. If not, I will have failed the test. The MRO will disclose my test results to Bluecrew.
I authorize the disclosure of my test results to Bluecrew, and any Bluecrew with whom I may be staffed or considered for assignment.
I release Bluecrew, its MRO, collection site and laboratory, and their agents from any liability they might otherwise have for the actions I am authorizing;
If I refuse to participate in a required drug screen, I will not be hired be placed on the requested assignment;
If I fail the drug screen, I will be ineligible to work for any Bluecrew client for 6 months following the failed drug screen; and
I understand that Government employers may impose added restrictions on pre-employment drug testing.
